Home Dialysis Care Coordinator, Bilingual Spanish and English Preferred – AKDHC


The Home Dialysis Care Coordinator, Bilingual Spanish and English Preferred provides comprehensive care management and coordinator of care for CKD and ESRD patients referred for Home Dialysis education and modality selection. The level of care coordinator provided is situational and appropriate based on patient or process need.


Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Lead the coordinator of home dialysis referral process, provide support, and work directly with referring the physician, the patient, dialysis centers, health care teams, and home dialysis centers as appropriate.
  • Provide education to patients and staff on home dialysis and treatment options, and vascular & PD access, early planning, and scheduling of vascular/PD access procedures.
  • Monitors all ESRD reports, CKD reports, and QA reports from administration and dialysis units.
  • Reviews the patient’s current vascular/PD access status and collaborates with the office and dialysis unit’s clinical staff in developing a plan of action for permanent access.
  • Coordinates timing of access placement based on Stage/GFR and patient need for dialysis.
  • Ongoing monitoring of the patient’s permanent access; communicates with staff in making sure any US, Limited Studies, and/or Interventions needed are scheduled.
  • Receives calls/faxes from patients, dialysis units, or physician offices needing to schedule patients for procedures, vessel mapping, ultrasounds, and permanent access placement.
  • Notifies the Nephrology and Surgical schedulers of any appointments/procedures that need to be scheduled.
  • Assists to schedule patients to outside vascular surgeons according to the patient’s access plan and insurance.
  • Follows up with patients regarding the surgical plan and provides education to patients that are being referred outside of AKDHC for access placement.
  • Performs direct follow-up with all outside surgeons on patient appointments and surgeries.
  • Obtain medical records, demographics, insurance information, all relevant clinical data on all patients referred to the home dialysis programs for CKD and ESRD patients.
  • Collaborate and Communicate potential issues regarding home dialysis referral to the patient physician, dialysis centers, and health care teams as appropriate.
  • Utilizes the appropriate technology platform and documents accurately and timely with an understanding of the requirements, expectations, and workflow. Understand home therapy referral information and documentation is both obtained from and submitted into multiple technology platforms (DocTalk, Athena, Emails, Sfax, eCube, Hospital Portals, and others as directed).
  • Utilize DocTalk and established workflows within DocTalk to identify and prioritize referral worklist.
  • Document timely referral status updates, progress, and comments in the DocTalk Home Therapy Module. Maintains ongoing tracking and appropriate documentation on referrals to promote dialysis team awareness of patient referral status.
  • Participates in the Home Therapy Team quality improvement process. Evaluates current processes, identifies the need for change, and implements change via process improvement projects to achieve optimal clinical, financial, operational, and satisfaction outcomes. Serves as a change agent by continually monitoring processes to identify opportunities for improvement. Implements change through a multi-disciplinary work group process.
  • Participates in meetings and educational programs with internal and external team members as appropriate.
  • Assists with various projects as assigned by direct supervisor.
  • Other duties as assigned.
  • Keeps supervisor informed on any patient, physician, and process issues, as well as overall opportunities for improvement.


Education/Experience:

  • High School diploma or general education degree (GED); or 3 to 6 months related experience and/or training; or equivalent combination of education and experience.
  • Previous dialysis or nephrology experience is highly preferred
  • Bilingual capabilities in Spanish and English highly preferred
  • Knowledge of Microsoft Office software, Spreadsheets, E-Mail, and Internet Software is helpful.
  • Basic Cardiac Life Support certification is required. For RN positions, a valid unrestricted RN license is required.
