To thrive/succeed as the Intake Coordinator, you should have:
- High school diploma or GED required
- Ability to type a minimum of 45 WPM
- Strong customer service and telephone communication skills
- Excellent organizational and time management abilities
- Experience using computers and common software applications
- Ability to work independently with minimal supervision
- Strong interpersonal and communication skills
- Ability to multitask and maintain attention to detail in a fast-paced environment
- Ability to learn company-specific technology and software programs quickly

Fluent verbal and written communication skills in English and Spanish.
Your responsibilities as the Intake Coordinator will include:
- Respond to incoming referrals and intake requests from referral sources, agencies, account managers, and patients
- Verify demographic, insurance, physician, and delivery information
- Coordinate admission and onboarding activities while ensuring appropriate services are provided
- Communicate with patients and caregivers to assess ongoing needs
- Obtain and process physician documentation and authorizations
- Coordinate with insurance providers regarding eligibility and authorization requirements
- Document and route communications in accordance with organizational policies
- Conduct patient follow-up and referral status communications
- Collaborate with internal teams, referral sources, and patients to resolve issues and improve service delivery
- Maintain compliance with company policies, procedures, and quality standards
- Perform additional duties as assigned
