Nevada Kidney Disease and Hypertension Center (NKDHC) is one of the largest Nephrology clinics in Nevada. We are experts in the treatment of Chronic Kidney Disease (CKD), End-Stage Renal Disease (ESRD), Acute Kidney Injury (AKI), Kidney Transplantation, and in creating and maintaining access to dialysis therapies. Our company is a professional medical association comprising physicians and other essential support staff. We are expanding and seeking Care Coordinators to join our teams in Las Vegas and Henderson.

As our Care Coordinator/Care Navigator, you will serve in a non-clinical, patient-focused role coordinating care and supporting patients with chronic kidney disease (CKD), including stages 4 and 5, as well as kidney transplant patients. You will focus on reinforcing education, coordinating care, conducting patient outreach, and navigating complex healthcare systems to ensure patients receive timely, appropriate, and continuous care. You will serve as the primary point of contact for patients, caregivers, nephrology providers, and transplant centers, helping patients understand their care pathway and identifying and addressing non-clinical barriers to care.
To be successful, you should have:
- Associate’s degree in healthcare administration, public health, social services, or a related field, or equivalent healthcare experience.
- Minimum of 2 years of experience in care coordination, patient navigation, case management, or a healthcare support role.
- Strong understanding of chronic disease management, preferably in nephrology or transplant services.
- Excellent communication, organizational, and interpersonal skills.
- Ability to manage multiple patients and priorities in a fast-paced environment.
- Proficiency with electronic health records and care tracking systems.
Preferred Qualifications:
- Experience in nephrology, transplant services, or specialty care practices.
- Care navigation or case management certification (non-clinical).
- Bilingual skills (preferred but not required).
- Familiarity with value-based care models and quality metrics in nephrology.
Your responsibilities as the Care Coordinator will include:
- Coordinate and navigate care for CKD stage 4–5 and kidney transplant patients, including appointments, referrals, testing, and milestone tracking across nephrology, dialysis, and transplant services.
- Support transplant patients by serving as a liaison with transplant centers, assisting with scheduling and documentation, and monitoring adherence to required follow-ups.
- Reinforce CKD and treatment education using provider-directed, practice-approved materials and assess patient understanding, escalating clinical questions as needed.
- Engage and outreach to patients and caregivers as a primary non-clinical contact, addressing barriers to care and connecting patients with appropriate resources and support services.
- Document and support program operations by maintaining EHR records, patient tracking tools, and contributing to quality improvement and multidisciplinary care activities within a non-clinical scope.
